# Terms of Service 

Please review our terms and conditions for using our services.

## Acceptance of Terms

By accessing and using the services provided by NWA Longevity Clinic, you agree to be bound by these Terms of Service. If you do not agree to these terms, please do not use our services.

## Services

NWA Longevity Clinic provides hormone optimization, weight management, IV therapy, and related wellness services. All services are provided under the supervision of licensed healthcare professionals.

## Patient Responsibilities

Patients are responsible for providing accurate and complete health information, following prescribed treatment plans, and attending scheduled appointments. Failure to do so may affect the quality of care provided.

## Payment & Billing

Payment is due at the time of service unless other arrangements have been made. We accept various forms of payment. Please contact our office for specific payment policies and available financing options.

## Cancellation Policy

We require at least 24 hours notice for appointment cancellations. Missed appointments or late cancellations may be subject to a fee. Please contact our office as soon as possible if you need to reschedule.

## Limitation of Liability

NWA Longevity Clinic and its providers shall not be liable for any indirect, incidental, or consequential damages arising from the use of our services. Individual results may vary, and no specific outcomes are guaranteed.

## Changes to Terms

We reserve the right to modify these terms at any time. Changes will be effective immediately upon posting. Continued use of our services constitutes acceptance of the updated terms.
